Is there hope for shoulder blade, protruding, & dislocated shoulder like this for 50 years? Pain ismuch to handle.

Shoulder and shoulder blade injured as a child, never repaired. Now I'm 60 and the bones grinding together is more pain than I can handle. It is keeping me from sleeping at night due to intense pain.
