Severe chest and back pain in right side for last 3 days gets worse during sleep or breathe out hard. should i go to ER?

Started 3 days ago. Experiencing pain in my chest and back on the right side.  The pain doesn't go away but sometimes becomes bearable.  It gets worse when I sleep and I can only stay asleep for a few hours before the pain wakes me up.  Breathing in doesn't seem to increase the pain but breathing out an coughing do.  Any idea what this could be.  The pain isn't stopping.  Should I go to the emergency room?
