What conditions will cause a skin crawling sensation?

My friend has had these symptoms following a trip to South America in July. Initially we thought he had bed bugs but  exterminator examinations and treatments. He tends to have the bugs crawling feeling under his feel, tip of his ears, on his scalp and sometimes by his genitles. I'm very concerned because I spend a lot of time around him but do not have any of his symptoms. Could this be the signs of a mental illness or could he have come kind of micro mites that I can't see.
