Glad you asked! You should be able to get a copy of the report by requesting in writing all reports or rating scales used in conducting an evaluation of your son's ADHD. These will be helpful in documenting his strengths and weaknesses, and well as targeting behaviors in order to discern progress made as a result of any treatment programs you undertake. If your son is over 18 years, you will need his permission, as well.
Over the years, I have frequently counseled parents to keep a folder on their child that includes copies of all outside evaluations, report cards, and notes from meetings with school personnel.
